Service Monitoring
The Harvey-Marion County CDDO conducts service monitoring to ensure that individuals receiving Home and Community Based Services (HCBS) are receiving quality supports that meet their needs and align with their person-centered service plans.
What Is Service Monitoring?
Service monitoring is a quality assurance activity conducted by the CDDO to review the services being delivered to individuals enrolled in HCBS waiver programs. The CDDO monitors services to verify that providers are delivering services as authorized, that individuals' rights are being upheld, and that health and safety needs are being met.
Monitoring visits may be announced or unannounced and can occur at the individual's home, day program, residential setting, or other service location. The CDDO uses standardized tools to document findings and follow up on any concerns identified during monitoring.

The Monitoring Process
Scheduling
The CDDO schedules monitoring visits based on the individual's service plan cycle, provider performance history, and any concerns that have been reported.
On-Site Review
CDDO staff visit the service location and use standardized review tools to assess service delivery, documentation, and the individual's experience.
Documentation
Findings are recorded using the appropriate monitoring tool. Any concerns or deficiencies are noted for follow-up.
Follow-Up
If issues are identified, the CDDO works with the provider and the individual's support team to develop a corrective action plan and verify resolution.
